+## Implementation notes

+- `apply_results` takes `core::state::PollOutcome` (`Ok`, `AuthFailed`,
+  `Failed`) instead of `usage::Error`, because the error type needs `net` and
+  `creds`, which return in Phase 03. The caller maps `AuthRequired` and
+  `TokenExpired` to `AuthFailed`, everything else to `Failed`, and logs the
+  transient error as before.
+- The cooldown lives in `UsageState::admit_alert`. Threshold alerts pass it
+  inside `apply_results`; `session_expired_alert` uses the same gate; the
