fix: address review feedback on proxy timeout

- Fix disable logic: timeout editable when proxy is stopped
- Remove hardcoded proxy_connect_timeout (60s is nginx default)
- Remove misleading '0 for no timeout' helper text
- Add min:1 validation for timeout value
